 Payroll Data Submission

Client should submit payroll requests to I & S Tax Service LLC by one of the following methods:  

• Fill up an online form (attach files there)
• Uploading required payroll information to the secure Client Portal 
• Email at: info@istaxservice.com
• Payroll Upload Form

For payroll services, complete the following documents and return them to us: 

• W4 Form  Employee’s Withholding 
• A-4 Form  Employee’s Arizona Withholding Election  
• I-9 Form   Employment Eligibility Verification
• 8655 Form Reporting Agent Authorization
• Employee Direct Authorization Form
